Ocular Manifestations of Autoimmune Disease
Rheumatoid arthritis, juvenile rheumatoid arthritis, Sjögren's
syndrome, the seronegative spondyloarthropathies, systemic lupus
erythematosus, multiple sclerosis, giant cell arteritis, and
Graves' disease are autoimmune disorders commonly encountered by
family physicians. These autoimmune disorders can have devastating
systemic and ocular effects. Ocular symptoms may include dry or red
eyes, foreign-body sensation, pruritus, photophobia, pain, visual
changes, and even complete loss of vision. Because a number of
these diseases may initially present with ocular symptoms,
physicians should maintain a high index of suspicion to make a
timely diagnosis.

Disease Manifestations and HLA Antigens in Psoriatic Arthritis in
Northern Sweden
In this cross-sectional study, clinical manifestations were more

HLA antigens. However, HLA antigens seemed to modify the expression
of the joint disease rather than being involved in joint disease
susceptibility.

Psoriatic spondyloarthropathy: A comparative study between HLA-B27
positive and HLA-B27 negative disease
Conclusions: The HLA-B27 antigen is not only important for PsSpA
susceptibility, but also determines some clinical features. This
antigen was associated with earlier age of psoriasis and arthritis
onset, bilateral sacroiliitis, and male gender. However, it was not
associated with either the severity or extension of the spondylitic
process or with functional impairment.

Functional Impairment in Spondyloarthropathy and Fibromyalgia
Conclusion. An almost similar functional decline was observed in
both SpA and FM patients when measured by the indices developed for
patients with AS and SpA. The specificity of BASDAI in measuring
disease activity in SpA was poor, as disease activity in FM was
rated higher than in SpA. (J Rheumatol 2002;29:1415-9)

Editorial: The Concept of Disease Modification in
Spondyloarthropathy - Journal of Rheumatology
There is now growing anatomical and therapeutic evidence to
introduce the concept of disease modification in SpA. This is based
on the fact that new and more potent antiinflammatory agents are
emerging and that sensitive imaging techniques such as MRI can be
used to measure regression of inflammation at the different sites
of disease. Confirmation that suppression of MRI determined
osteitis prevents subsequent bone ankylosis could herald a new era
for the appraisal of therapies in SpA, whereby drugs will be
initiated based on the evidence for their effect at the primary
site of disease in SpA rather than on a successful track record in
the therapy of RA, as has been the case until now.
